Installer Application Questionnaire

This application is how we evaluate installers for the MacroAir Certified Installer Program. Answer every question as specifically as you can — installations, licenses, safety programs, and reference details are independently verified.

Before you start: Have your license numbers, insurance carrier and coverage amounts, most recent TRIR (Total Recordable Incident Rate) and EMR (Experience Modification Rate) figures, and three professional references on hand. Fields marked * are required to submit. A few questions carry extra weight in our review — those are flagged Key criterion.

1. Company Info

3. Insurance & Bonding

6. Knowledge Assessment

Per OSHA, what is the minimum clearance between the lowest HVLS fan blade and the floor?*

Be specific and sequential. Strong answers cover: pre-inspection, structural verification, lockout/tagout, wiring, torque verification, blade clearance check, low-speed spin test, escalation path, and documentation.

7. Equipment & Capacity

9. Program Fit

10. Attestation

MacroAir requires all quote responses returned within 48 hours of receipt. Installers who do not respond within this window will be removed from active lead distribution. I acknowledge and commit to this requirement.*

I confirm all information provided in this application is accurate. I consent to MacroAir's verification of all licenses, insurance, EMR (Experience Modification Rate), TRIR (Total Recordable Incident Rate), and references. I agree to complete factory training before performing any certified installation, and to annual re-certification.*
